主动脉夹层动脉瘤62例外科治疗分析

Surgical treatment of 62 cases of dissecting aneurysm of aorta

目的总结主动脉夹层动脉瘤的外科治疗经验。方法回顾分析体外循环下治疗62例主动脉夹层动脉瘤患者资料。结果DeBakeyⅠ型48例、DeBakeyⅡ型11例、DeBakeyⅢ型3例采取体外循环手术,同期体外循环下植入大血管腔内覆膜支架25例。全组患者随访期间死亡3例,总死亡率4.8%。结论结合外科手术和支架植入方法能提高主动脉夹层动脉瘤治疗疗效。

Objective To investigate the surgical treatment experience of dissecting aneurysm of aorta, surgical procedures and follow - up outcome. Methods Among 62 cases of dissecting aneuryms of aorta, there were 48 cases of DeBakey Ⅰ type, 11 cases of DeBakey Ⅱ type and 3 cases of DeBakey Ⅲ type. All patients were operated on under cardiac pulmonary bypass （CPB） procedure. The lesioned aort was replaced according to the involed positions. Twenty -five patients were treated with a self - expanding polyester - covered stent - graft during the surgical procedure. Results During the follow - up period, there were 3 deaths （4.8%）. Conclusion Stent - grafts are an important means of treatinz dissecting aneurysm of aorta during the surgical procedure.
