洗涤汇集白膜层血小板的临床应用 被引量:2

摘要 目的观测汇集白膜层(BC)法制备的洗涤汇集血小板(洗涤汇集BC-PC)的临床输注效果。方法400ml/袋新鲜全血在4~6h内分离出的BC在22℃静置过夜,6袋同血型的BCs混合制备的混合BC-PC再用生理盐水洗涤,制备的洗涤混合BC-PC用血细胞计数仪测定血小板含量和红细胞残留量、用Nageotte白细胞计数板测定白细胞残留量,用比浊法测定最大聚集率,用流式细胞计数仪测定CD41和CD62p阳性表达率,同时检测洗涤产品的pH值和血浆蛋白清除率;通过测定输注洗涤血小板患者的血小板校正计数增加值(CCI)来确定血小板的输注效果,针对有输血性过敏反应史或血小板输注无效患者输注洗涤血小板后是否发生输血性不良反应来确定纠正输血性不良反应的效果。结果患者输注洗涤血小板后1hCCI>10占87.5%,没有发生输血性不良反应。结论患者输注洗涤汇集血小板能够达到血小板的输注效果和纠正、预防输注血小板引起的输血性过敏反应、血小板输注无效及其他输血性不良反应。
