摘要
目的探讨尿脱落细胞Tenascin-c(TN-C)基因检测在膀胱癌诊断中的价值。方法采用逆转录聚合酶链反应(reverse transcription polymerase chain reaction,RT-PCR)半定量法检测32例膀胱癌患者、22例泌尿生殖系良性疾病患者和5例健康志愿者清晨新鲜尿尿脱落细胞Tenascin-c mRNA的表达水平,并与尿脱落细胞学检查相对照。结果用RT-PCR法检测尿脱落细胞Tenascin-c mRNA用于诊断膀胱癌的敏感性为81.3%,高于尿脱落细胞学检测(37.5%)(P<0.05),不同分化程度及不同临床病理分期膀胱癌患者之间尿脱落细胞Tenascin-c mRNA的阳性表达率的差异有统计学意义(P<0.05)。结论尿脱落细胞Tenascin-c mRNA的检测可作为膀胱癌早期诊断、复发监测的一项极其灵敏的方法,并有助于肿瘤分级分期的判断。
Objective To study the significance of the detection of Tenascin-c mRNA in exfoliated urothelial cells for the diagnosis of bladder cancer. Methods The fresh urine voided firstly in. the morning were collected from 32 patients with bladder cancer, 22 patients with benign urogenital diseases and 5 healthy volunteers. Tenascin-c mRNA expression was studied by mean of reverse transcriptase polymerase chain reaction (RT-PCR) with semiquantitative analysis in urine exfoliated cells, and controlled experiment was conducted by urinary cytology analysis. Results The sensitivity of Tenascin-c mRNA for urine exfoliated ceils was 81.3 %, which was signicantly higher than that of cytology examination (37.5 % ) (P〈0.05). There were significant different positive expression rates of Tenascin-c mRNA in urine exfoliated ceils from the bladder cancer patients with differrent grades or clinicopathological stages (P〈0.05). Conclusion RT-PCR assay to detect Tenascin-c mRNA in urine exfoliated cells is an extremely sensitive method for the early diagnosis and postoperative inspection of bladder cancer. It can help to identify the grade and stage of bladder cancer.
