大型带蒂肌皮瓣修复口腔颌面肿瘤术后缺损

Construction of Oral and Maxillofacial Defect after Operation with Massive Pedicle Myocutaneous Flap

目的应用大型带蒂瓣修复口腔颌面复杂性缺损。方法7例额瓣和2例胸大肌肌皮瓣分别修复9例口腔颌面恶性肿瘤术后缺损,缺损最大面积6cm×5cm,最小面积4cm×4.5cm;修复洞穿性缺损所用折叠皮瓣最大面积达5cm×11cm。结果额瓣转移术后早期表现瘀血;胸大肌肌皮瓣早期表现缺血;9例肌皮瓣全部成活,形态和功能满意。结论额瓣或胸大肌肌皮瓣修复口腔颌面部大面积、复杂性缺损是比较简便、实用的方法。

Objective To repair oral and maxillofacial complex defect with massive pedicle muscle and skin flap.Methods Total 7 cases of forehead flap and 2 cases of pectoral major myocutaneous flap were seperately used to repair 9 patients who had been operated for vicious tumor.The most area of the defect is 6 cm×5 cm,and the least is 4cm×4.5cm in 9 cases.The most area of folded flap with repairing hole defect reaches 5 cm×11 cm.Results Blood clot appear in early stage after forehead flap transferred.Lack of blood appear in early stage for pectoral major myocutaneous flap transfering.The flaps of 9 cases are all surviving.Shape and function are entirely satisfied.Conclusion Forehead flap and pectoral major myocutaneous flap can be applied to repair larger,complex oral and maxillofacial defect.
