腹腔镜普外科妇科联合手术的临床应用

Clinical study on combined laparoscopic of general surgery and gynecology

目的:探讨腹腔镜普外科、妇科联合手术的临床可行性及效果。方法:21例普外科、妇科腹腔镜联合手术,其中腹腔镜下乙状结肠代阴道治疗先天性无阴道10例,腹腔镜胆囊切除(LC)+腹腔镜妇科手术11例。结果:手术均获成功,无中转开腹,术后疼痛轻微。无手术并发症发生。手术时间70￣512分钟、平均199.1分钟(其中腹腔镜下乙状结肠代阴道术平均296.3分钟);术中出血5￣800毫升、平均166.8毫升;术后肠功能恢复时间4￣36小时、平均24小时;术后住院日2￣15天,平均住院日7.6天。结论:普外科、妇科联合腹腔镜手术安全、有效、经济、可行,具有广阔的应用前景。

Objective To study the clinical practicability and efficacy of combined multi-disciplinary laparoscopic surgery.Methods Twenty-one patients were undergone combined multi-disciplinary laparoscopic of general surgery and gynecology. Ten patients were performed laparescopically assisted sigmoid transplantation. Eleven patients were performed laparoscopic cholecystectomy plus laparoscopic gynecological operation.Results Twenty-one patients were performed successfully.There was no conversion to open surgery. The pain after surgery was slight and no complication occurred.The operation time was 70-512 min（average 199.1 min）;bleeding range from 5 to 800 ml（average 166.8 ml）;the recovery of bowel function was 4-36 h（average 24h） and the time of hospitalization after surgery was 2-15d （average 7.6d）. Conclusions Combined genecological and general surgical laparoscopic surgery can be performed safely , effectively ,practically and economically. The modality is of high value and will be utilized widely.
