摘要
目的:探讨人巨细胞病毒(human cytomegalovirus,HCMV)感染孕妇后致宫内感染的机制。方法:检测15例血清HCMV IgM(+)早孕妇女绒毛中HCMVmRNA的表达;检测HCMVmRNA表达阳性的绒毛原位杂交蜕膜中诱生型一氧化氮合酶(induced ni-tric oxide sythnase,iNOS)mRNA的表达,用免疫组化技术检测细胞周期蛋白(Cyclins)D、E、B、A蛋白的表达,用生化方法测定绒毛组织中的超氧化物歧化酶(superoxide dis-mutase,SOD)、丙二醛(malondialdehyde,MDA);以同期血清HCMV IgM(-)、绒毛HCMVmRNA(-)的15例孕妇绒毛为对照组。结果:(1)15例血清HCMV IgM(+)孕妇中5例绒毛组织HCMVmRNA阳性,宫内传播率为33.33%;(2)5例HCMV-mRNA阳性绒毛Cy-clin D、Cyclin E表达发生率均较对照组高(80%vs 7.1%;80%vs 15.5%);而两组均无Cyclin A及Cyclin B表达;(3)HCMV-mRNA阳性蜕膜中iNOS-mRNA呈强表达,其吸光度为0.5021±0.0189显著高于对照组的0.1860±0.0119,差异有统计学意义(P<0.05);(4)HCMV mRNA阳性绒毛中SOD、MDA均高于对照组(178.35±13.25 vs 102.27±10.12;22.18±5.02 vs 10.15±4.56),(P均<0.05)。结论:孕早期绒毛活动性HCMV感染可导致Cyclin异常表达,NO、SOD、MDA对Cyclin的异常表达起了重要的作用。
Objective:To investigate mechanism of teratogenesis in pregnant women with infected by HCMV ( human cytomegalovirus ). Methods: HCMV-IgM in maternal sera was evaluated by ELISA and HCMV-mRNA in chorionic villi was determined by RT-PCR. For subjects with both HCMV-IgM ( + ) and HCMV-mRNA ( + ), iNOS-mRNA in their decidua was detected through hybridization in situ. Furthermore, cyclins ( D, E, B, A ) were tested by immohistochemical test and SOD and MDA were determined by biochemical assay in chorionic villi. Meanwhile,15 women were studied as control who had healthy pregnancy and HCMV-IgM(-) in sera,HCMV-mRNA(-) in chorionic villi. Results: (1)Fifteen pregnant women were HCMVIgM ( + ) in serum of whom five also had result of HCMV-mRNA ( + ) in chorionic villi. The intrauterine transmission rate was 33.33%. (2) Five patients with HCMV-mRNA( + ) in chorionic villi had more frequency of cyclin D and E than that of control group ( 80% vs 7.1%, 80% vs 15.5% ). However,cyclin A, cyclin B wasn't detected in both of them. (3) iNOS-mRNA had statistically higher level in decidual samples with HCMV-mRNA( + ) than that of control group (0.5021 ±0. 0189 vs 0. 1860±0.0119 ,P 〈 0.05 ). (4)Chorionic villi with HCMV mRNA( + ) showed elevated levels of SOD and MDA with statistical difference( P 〈 0.05 ) in comparison of control group. Conclusion: Expressions of eyelins in ehorionie villi are altered in active HCMV infection during early pregnancy in which elevated-level NO, SOD and MDA may play important roles.
