河南地区有偿供血者HIV母婴传播危险因素调查 被引量:2

摘要 目的探索河南地区艾滋病病毒Ⅰ型(HIV-1)母婴传播率和影响因素,为开展阻断工作提供背景资料。方法以HIV阳性并通过母婴传播导致子女感染HIV的母亲为对象,以未导致子女感染的母亲为对照,分析影响母婴传播的危险因素。结果采取母婴阻断措施前后的母婴传播率分别为23.02%(32/139)、5.00%(1/20)。影响母婴传播的显著危险因素为:CD4细胞数〈419个/μl(OR=30.31,95%CI 9.59~124.88)、胎次(第1胎/≥2胎:OR=3.16,95% CI 1.22~8.33)、产程≥4小时(OR=24.27,95%CI 7.69~99.60)。结论Logistic回归分析显示,产程≥4小时(OR=24.24,95%CI 7.87~74.64)是影响母婴传播的独立危险因素。 Objective To study HIV infection and its risk factors to mother to child transmission(MTCT)in Henan, so as to provide reliable background information for the development of appropriate interventions. Methods HIV positive mothers who transmitted HIV to their babies were selected as the case group, and those without transmission were selected as the control group. The risk factors to MTCT were evaluated based on the case-control study approach. Results The rates of MTCT before and after the interventions were 23.02 % (32/139)and 5.00 % (1/20) respectively. The results of the univariate analysis showed that the risk factors that might influence MTCT were immune conditions (OR = 30.31, 95% CI 9.59 - 124.88) ,the times of child delivery(first or second delivery)( OR= 3.16,95% CI 1.22 - 8.33) ,and the process of delivery(≥ 4 hours) ( OR = 24.27,95 % CI 7.6 - 99.60). Conclusion The results of the logistic regression analysis indicate that the process of delivery ( OR = 24.24,95 % CI 7.8 74.64)is an independent risk factor to MTCT.
