摘要
目的通过观察骨关节炎软骨细胞人类白细胞DR抗原(HLA-DR)表达及软骨细胞体外刺激自体外周血单核细胞(PBMC)增殖反应,探讨软骨细胞的抗原呈递作用。方法依据1986年美国风湿病学会(ARA)制定的膝骨关节炎诊断标准,收集16例膝骨关节炎患者软骨标本及5例股骨颈骨折患者软骨标本,进行软骨细胞培养及白细胞介素-1α(IL-1α)刺激后软骨细胞培养,采用流式细胞技术检测HLA-DR表达;将软骨细胞与自体PBMC共同培养,采用甲基噻唑基四唑(MTT)法测定PBMC增殖反应。结果对照组和骨关节炎组HLA-DR的表达值(%)分别为0.61±0.18和1.82±0.28;经IL-1α刺激后,两组HLA-DR表达均增高,分别为1.62±0.25和3.89±0.43,骨关节炎组均高于对照组(P〈0.01)。对照组和骨关节炎组软骨细胞吸光度(A)(490)值分别为0.162±0.018和0.452±0.035,骨关节炎组明显高于对照组(P〈0.001)。结论膝骨关节炎软骨细胞能够表达HLA-DR,且经IL-1α刺激后HLA-DR表达增高;骨关节炎软骨细胞有刺激PBMC增殖的潜能,为其发挥提呈抗原作用提供更多依据。
Objective To approach the antigen presenting role of the chondrocytes through observing the HLA-DR expression on chondrocytes and the peripheral blood monouclear cells proliferation stimulated by autologous chondrocytes. Methods According to the knee osteoarthritis diagnostic criteria instituted by American Rheumatism Association(ARA) in 1986, articular cartilage tissue of knee joint from 16 osteoarthritis (OA) and articular cartilage tissue of collum femoris from 5 femoral neck fracture patients were collected. Then these chondrocyte were cultured with or without IL-1α. Expression of HLA-DR was assessed by flow cytometry. Peripheral blood mononuclear cells were co-cultured with autologous chondrocytes, and their proliferative response was assessed by methyl thiazolyl tetrazolium (MTT). Results The HLA-DR expression in OA group and in control were 1.82±0.28 and 0.61 ±0. 18 respectively, and if stimulated by IL-1α, the HLA-DR expression were higher (3.89±0.43) in OA group and 1.62± 0.25 in control (P±0.01). The A490 of OA group (0. 452±0. 035) was higher than of the control (0. 162±0. 018) (P〈0. 001). Conclusions Knee OA chondrocyte can express HLA-DR, and HLA-DR expression will be higher if the chondrocyte was stimulated by IL-1α. OA chondrocyte have the potential ability to stimulate the response of the PBMC, which provides more evidence that chondrocyte have the role of antigen presentation.
