全麻药对胃癌患者围手术期血清白细胞介素-6和皮质醇的影响 被引量:1

摘要 目的监测胃癌根治术病人围手术期血清白细胞介素-6、皮质醇的变化,探讨异丙酚、依托咪酯和异氟醚对患者围手术期应激反应的影响。方法30例拟行胃癌根治术患者,分为异丙酚组(A组)、依托咪酯组(B组)和异氟醚组(C组),分别于麻醉前5min(T1)、麻醉后60min(T2)、手术结束后30min(T3)、术后24h(T4)和术后48h(T5)采集静脉血,用放射免疫分析技术测定血清IL-6、皮质醇含量;另选10例健康成年人为对照组(D组)。结果胃癌根治术患者在麻醉前己处于应激状态;三组病人血清IL-6含量随着麻醉时间的延长逐渐升高,在T4时点达到最高峰,以后逐渐下降;T3、T4时点A、C组IL-6低于B组(P〈0.05),T5时点A组IL-6低于B、C组(P〈0.01)。随着麻醉的延长,A、C组皮质醇逐渐升高,在T3时点达到最高峰,B组逐渐下降,在T4时点达到最低;在T2-T4时点B组皮质醇低于A、C组(P〈0.01),T5时点A组皮质醇低于B组(P〈0.01),T2-T5时点A组皮质醇低于C组(P〈0.05)。结论麻醉和手术创伤可加重围手术期的应激反应,导致IL-6、皮质醇分泌增加;依托咪酯较异丙酚和异氟醚更能够抑制皮质醇的合成和分泌,异丙酚较依托咪酯和异氟醚更能抑制患者围手术期的应激反应;异丙酚全静脉麻醉是胃癌根治术较好的麻醉选择。
