置宫内节育器与细菌性阴道病的关系

Installation of Intrauterine Device and Bacterial Vaginitis in Women

目的探讨置宫内节育器(IUD)与细菌性阴道病(BV)的关系,同时为安全使用IUD提供依据。方法采用唾液酸酶活性法对观察组328例置IUD妇女进行了BV的检测,并以同期体检的74例未置IUD妇女作为对照。从置IUD的类型、置IUD后是否出血、有无合并其他病原体感染等方面对两组妇女进行了对比分析。结果置IUD妇女BV患病率为29.6%,对照组为13.5%,两组间差异有统计学意义(P<0.01)。三种不同类型IUD使用者的BV患病率间差异无统计学意义(P>0.05)。出血组与未出血组及对照组BV患病率间差异均有统计学意义(P<0.01),未出血组与对照组BV患病率间差异无统计学意义(P>0.05)。BV组合并感染率为82.7%,非BV组合并感染率为34.5%,两组间差异有统计学意义(P<0.01)。结论放置IUD与BV的发生有关。置IUD后出血会增加BV的危险性。置IUD妇女中BV患者合并其他病原体感染的概率升高。

Objective To discuss the relationship between intrauterine device （IUD） and bacterial vaginitis （ BV）, and to provide a basis for the safe use of IUD. Methods BV was detected for 328 women with IUD （ trial group） and 74 women without IUD （control group） by means of sialidase activity. Then comparison was made between the two groups in such aspects as types of IUD, whether there is bleeding after IUD, and whether there is complication of infection by some pathogens. Results The positive rate of BV was 29. 6% in the trial group, and was 13. 5% in the control group, with a significant difference （ P 〈 0.01 ）. In BV positive rate there was no significant difference among the three types of IUD （ P 〉 0. 05 ） ; there was a significant difference in BV positive rate among the bleeding group, non - bleeding group and the control group （ P 〈 0. 01 ） ; and there was no significant difference between the non - bleeding group and the control group （ P 〉 0. 05 ）. The rate of complicated infection in BV group was 82. 7% , and in non BV group was 34. 5%. with a significant difference （ P 〈0. 01 ）. Conclusion The installation of IUD is associated with occurrence of BV. Bleeding after IUD could increase the incidence of BV. The rate of complicated infection by other pathogens for IUD women with positive BV increases.
