摘要
目的探讨凋亡调控因子Survivin、半胱氨酸天门冬氨酸蛋白酶-3(caspase-3)及细胞增殖标记指数(PCNA)在卵巢上皮性肿瘤中表达的临床意义。方法采用免疫组织化学技术,检测2006年1月至2006年12月哈尔滨医科大学第一临床医学院收治的40例卵巢上皮性癌、10例交界性卵巢上皮性肿瘤组织及10例卵巢良性上皮性肿瘤中Survivin、caspase-3和PCNA的蛋白表达,并结合临床病理特征进行分析。结果Survivin、caspase-3在卵巢上皮性肿瘤中表达呈负相关;Survivin和PCNA的蛋白表达呈正相关;Survivin在卵巢上皮性癌中表达率72.5%(29/40),且表达水平高于卵巢良性上皮性肿瘤(0)和交界性卵巢上皮性肿瘤(40%)(P<0.05)。caspase-3在卵巢上皮性癌中表达率0.5%(2/40),且表达水平远远低于卵巢良性上皮性肿瘤和交界性卵巢上皮性肿瘤(P<0.01)。结论Survivin可能抑制caspase-3在卵巢癌中表达,导致卵巢癌细胞增殖,凋亡受阻,肿瘤形成;Survivin基因的表达可能发生在卵巢癌恶性转化的早期;Survivin的表达与瘤细胞分化、增殖密切相关,并且其高表达可能会作为预后不良的标志;caspase-3在卵巢癌中极低表达,有望成为卵巢癌良恶鉴别新的指标。
Objective To investigate the expression of apoptosis regulatory factor Survivin, caspase-3 and PCNA in 40 cases of epithelial ovarian tumor and its clinical significance. Methods To detect the expression of proteins Survivin, caspase-3 and PCNA in 40 cases of epithelial ovarian cancer, 10 cases of borderline epithelial ovarian tumor and 10 cases of benign epithelial tumor;analysis is carried out in conjunction with clinical pathological features. Results Survivin and caspase-3 are of negative correlation in epithelial ovarian tumor; the protein expression of Survivin and PCNA are of positive correlation; the expression rate of Survivin in epithelial ovarian cancer was 72. 5% (29/40) , and the expression level was higher than epithelial ovarian tumor and borderline epithelial ovarian tumor ( P 〈 0. 05 ). The expression rate of caspase-3 in epithelial ovarian cancer was 0.5% (2/40) , and the expression level was much lower than epithelial ovarian tumor and borderline epithelial ovarian tumor ( P 〈 0. 01 ). Conclusion The expression of caspase-3 is inhibited by Survivin, which results in the ovarian cancer cells multiplication, apoptosis obstruction and tumor generation ; the expression of Survivin gene happens in the early stage of ovarian cancer malignant transformation; the expression of Survivin is closely correlated with tumor cell differentiation and proliferation, and its high expression can he the identification mark of unfavourable prognosis; the expression level of caspase-3 in ovarian cancer is quite low, which makes it a new index to identify the benign and malignant ovarian tumor; the qualitative and quantitative analysis of Survivin, caspase-3 and PCNA may become the index of curative effect of drugs.
