200例正常胎儿卵圆孔和卵圆孔瓣彩色多普勒超声特征分析 被引量:7

摘要 目的建立正常胎儿卵圆孔、卵圆孔瓣二维超声和血流动力学特征。方法随机对200例孕18~42周正常胎儿的卵圆孔、卵圆孔瓣二维超声和彩色多普勒频谱特征进行分析,并根据卵圆孔瓣超声表现把卵圆孔瓣分为弧型、单瓣型、双瓣型和游离型;根据卵圆孔的血流方向分为右向左分流型和双向型。结果符合条件的190例胎儿参与本研究统计和分析,其中85例胎儿卵圆孔瓣表现为弧型,73例为单瓣型,23例为双瓣型,9例为游离型。190例胎儿卵圆孔血流均为右向左分流,血流频谱表现为双峰,波峰s发生在心室收缩期,波峰D发生在心室舒张早期。结论超声下卵圆孔瓣可表现为弧型、单瓣型、双瓣型和游离型;彩色多普勒超声下卵圆孔血流为右向左分流,其频谱表现为双峰,波峰S发生在心室收缩期,波峰D发生在心室舒张早期。 Objective To establish the normal sonographic characteristics of foramen ovalve valve (FOV) and flow pattern of foramen ovalve(FO). Methods FOV and FO of 200 normal fetuses were studied using a combined two-dimensional real-time and color Doppler ultrasonography. FOV was catalogued into arc shape, single valve shape, double valve shape and dissociated valve according to ultrasonographic appearance. Transforamenal flow was calssified into right-to-left and bidirection. The sonographic characteristics were investigated by comparison with anatomic features of FOV. Results Among the 200 fetuses, 190 were included in this study. The number of arc shape, single valve shape, double valve shape and dissociated valve was 85,73,23,9 respectively. Transforamenal flow showed a biphasic flow waveforms of right-to-left,with S wave occurring at the ventricular systolic and D wave occurring at the early ventricular diastolic phase and no left-to-right flow was found. Conclusions The sonographic appearances of FOV maybe arc shape, single valve shape, double valve shape or dissociated valve in normal fetuses. Transforamenal flow showed a biphasic waveforms of right-to-left, with S wave occurring at ventricular systolic and D wave occurring at the early ventricular diastolic phase.
