甘肃省神经外科状况的调查报告

The epidemiologicai features of in hospital patients of neurosurgery department in Gansu Province

目的统计分析甘肃省神经外科的床位、医师数量、住院患者基本分布及流行病学特点，为神经外科疾病的预防及诊治提供依据。方法对省内独立建制的神经外科发放表格，统一汇总、统计分析。结果17家三级医院和7家二级医院参与调查，平均每位神经外科医师年收治患者71．18位，每10万人有0．59名神经外科医师和2．71张神经外科床位。神经外科患者及手术数量逐年增多。颅脑肿瘤、脑动静脉畸形、动脉瘤及癫痫的外科诊治主要集中在省级大医院，下级医院主要诊治颅脑外伤、高血压脑出血等。结论甘肃省神经外科实力薄弱，尤其是基层医院，神经外科疾病逐年增加，给社会和家庭带来沉重的负担。

Objective To explore the number of hospital bed and neurosurgeon, the distribution and the epidemiology features of in hospital patients at neurosurgery department in Gansu Province during the past 3 years, and to provide the evidence for prevention and diagnosis diseases of neurosurgery. Methods The investigation forms were distributed to neurosurgery department, which was built independently in province. The data was collected by Director of each neurosurgery department and statistical analyzed by neurosurgery department of the 2nd hospital of Lanzhou university. Results 17 third grade hospitals and 7 second grade hospitals were looked into. It was demonstrated that there are 702 hospital beds and 153 neurosurgeons, 71.18 patients were treated by each neurosurgeon per year, 0.59 neurosurgeons and 2.71 beds took care of 100 000 people. The number of in hospital patients and the cases of operation from 2004 to 2006 were increasing year by year. The maximal kind of diseases was cerebral trauma, brain tumor and cerebrovaseular disease, but the brain tumor was the highest ratio in operation cases, cerebral trauma and cerebrovaseular disease lied to the 2nd and 3rd places. Surgical diagnosis and therapy for brain tumor, AVM, aneurysm and epilepsy were concentrated mainly at the province hospital. Cerebral trauma, hypertensive cerebral hemorrhage and skull defect could be operated at the district hospital, and the cerebral trauma was the main disease that was cured at the county hospital. The therapy for Parkinson＇s disease was not carried out in our province. Conclusion It is gradually increasing at the number of in hospital patients and the cases of operation in neurosurgical department. Cerebral trauma, brain tumor and cerebrovascular diseases obviously have higher incidence which causes heavy burden to society and family. The therapy and diagnosis of cerebrovascular disease, epilepsy and Parkinsong disease were the weakness at neurosurgery in the whole province. The neurosurgery strength of basic hospital is frail. It deserves government and society to pay attention to the neurosurgical diseases.
