16层CT多种后处理方法在主动脉夹层动脉瘤中的应用

摘要 目的:探讨16层螺旋CT多平面重组(miltiplanar reformation,MPR)、曲面重组(curved planar reforma- tion,CPR)及容积再现(Volume Rendering,VR)在主动脉夹层动脉瘤中的应用价值。方法:对45例主动脉夹层动脉瘤的患者进行螺旋CT容积扫描,然后在工作站上进行MPR、CPR、VR成像,结合轴位扫描图像,观察这三种重建图像对主动脉夹层动脉瘤显示情况及主要动脉分支受累情况。结果:MPR对判断真假腔、内膜瓣、破口入口有帮助;CPR显示主动脉夹层动脉瘤整体效果较好;VR整体观最好,但显示内膜瓣、破口入口不如前两者。结论:MPR、CPR、VR这三种方法结合轴位图像,能够立体直观、整体地显示主动脉夹层动脉瘤情况,对临床医生决定治疗方案提供了依据。 Objective To investigate the clinical applicable value of helical CT mutiplanar reformation( MPR), curved planar reformation(CPR) and volume Rendering in patients with dissecting aneurysm of aorta. Methods Fourty-five cases with dissecting aneurysm of aorta underwent helical CT volume scans. After scanning, the raw data were transferred to workstation and then processed with MPR, CPR and VR. According to the MPR, CPR, VR images and transaxial images, the low density intimal flap,the entrance, the true channel and flase channel, the involvement of collateral branches of the aorta were observed. Results MPR images were helpful in diagnoses of the low density intimal flap, the entrance, the true channel and flase channel, CPR can show the whole disorder; VR was good in showing whole aorta stereoscopically, although VR was superior to others in display details in the aorta lumen. Conclusion Combination of axial images, MPR, CPR and VR can provide an advantage of showing dissecting aneurysm of aorta more accurately and stereoscopically. These techniques have an important value to surgeons to choose therapeutic method and surgical approach.
