异丙酚复合瑞芬太尼全凭静脉麻醉的临床观察

摘要 目的:观察异丙酚复合瑞芬太尼全凭静脉麻醉的临床效果。方法:选择ASAⅠ级~Ⅱ级择期行腹部手术患者80例,随机分成静吸复合组(C组)和异丙酚-瑞芬太尼组(R组)。每组40例,两组均以力月西0.07 mg/kg、芬太尼2μg/kg~4μg/kg、异丙酚1 mg/kg~1.5 mg/kg、维库溴胺0.1 mg/kg静脉诱导行气管插管,术中C组以异氟醚、氧化亚氮维持麻醉,间断追加芬太尼、维库溴胺。R组以异丙酚4 mg/(kg·h)~12 mg/(kg·h)和瑞芬太尼0.2μg/ (kg·h)~2μg/(kg·min),用微量泵持续静脉输注,间断追加维库溴胺。术毕自主呼吸恢复,清醒后拔管。分别记录患者术中血流动力学变化及停药后患者清醒时间、拔管时间和定向力恢复时间。结果:两组患者术中平均动脉压(MAP)、心率(HR)与术前比较没有明显变化。C组术后清醒时间、拔管时间均晚于R组。结论:异丙酚复合瑞芬太尼全凭静脉组患者术中血流动力学稳定,术后恢复快,苏醒质量好,同时可以减少手术室污染,是一种较为理想的麻醉方法。
