非功能性肾上腺肿瘤的诊断与外科治疗(附26例报告)

Diagnosis and treatment of non-functional adrenal gland tumors-A report of 26 cases

报告外科治疗非功能性肾上腺肿瘤26例,其中仅7例有症状,19例无症状与体征,是在体检中被B超偶然发现。文中指出,非功能性肾上腺肿瘤的定位诊断需依靠B超、CT、MRI及肾上腺血管造影,并强调术前都要作系统的内分泌功能检查,以鉴别其有无功能,尤其是要与嗜铬细胞瘤相鉴别。肿瘤直径大于3.5cm应外科手术治疗;直径小于3.5cm的肿瘤虽可在B超、CT的严密监视下进行随访观察,但对青壮年患者,因其肿瘤会继续生长增大,亦应手术治疗。

Of 26 cases of non-functional surgically treated adrenal gland tumors,7 were found having symptoms and 19 non-symptoms. The latter ones were incidentallyy found by B-ultrasonography on routine physical examination. The diagnosis location of non-functional adrenal gland tumors was dependent upon B-ultrasonography ,CT,MRI and adrenal angiography. It was stressed that systematic endocrinous examination should be performed before the operation in order to confirm whether the tumors were functional or not,and. especially to distinguish them from adrenal pheochromocytoma. Open operation should be undertaken for tumors over 3.5 cm in diameter. However, the patients with the tumor less than 3. 5 cm in diameter might be followed up with B-ultrasonography and CT scanning. As the tumors less than 3. 5 cm in young and middleage persons would grow up continuously .open operation should also be made on these patients.
