摘要
目的探讨Ki-67在脂肪肉瘤病理诊断与鉴别诊断中的意义。方法应用组织芯片技术和免疫组织化学Envision方法检测41例脂肪肉瘤和20例脂肪瘤中Ki-67蛋白和S-100蛋白的表达情况。结果Ki-67表达于脂肪(母)细胞的胞质(膜),Ki-67在脂肪肉瘤中的阳性率为78%(32/41),S-100在脂肪肉瘤中的阳性率为46%(19/41),前者明显高于后者,两者差异有统计学意义(P<0.05);在分化差的脂肪肉瘤中Ki-67的阳性率(69%)高于S-100(31%),两者差异有统计学意义(P<0.05);Ki-67在脂肪肉瘤与脂肪瘤中的表达的阳性率差异无统计学意义(P>0.05)。结论Ki-67在脂肪性肿瘤中的表达有较强的敏感性和特异性,可以作为脂肪性肿瘤的特异性标记,特别是在分化差的脂肪肉瘤中,配合标记CD34显示肿瘤中的毛细血管网,可以提高脂肪肉瘤的诊断准确率。
Objective To investigate the Ki-67 protein expression and its significance in pathological diagnosis and differential diagnosis of liposarcoma, Method's Formalin-fixed and paraffin-embedded materials from 41cases of liposarcoma and 20 cases of lipoma were studied by Envision immunohistochemistry and tissue microarray technology for Ki-67 protein and S-100 protein. Results Ki-67 expressed in cytoplasm (or cytolemma) of lipocyte (or lipobla.st). The expre.ssion rates of Ki-67 and 8-100 in liposarcoma were 78.0% and 46.3% respectively, the difference in expression rates between Ki-67 protein and S-100 was statistically significant (P 〈 0.05); The expression rate of Ki-67 in poorly differentiated liposarcoma was 69.2% and 8-100 were 30.8% The difference in expression rates of Ki-67 protein in these two groups of lipoma and liposarcoma showed no statistical significance (P〈 0.05). Conclusion Ki-67 has comparative powerful sensitivity and specificity in lipo-tumors. Especially in poorly differentiated liposarcoma, marking CD34 to show tumor's capillary network will raise accurate rates of diagnc,sis of liposacoma; Ki-67 is of no use in discrimination of tumor's malignancy.
