64层螺旋CT血管成像诊断胚胎型大脑后动脉及与DSA诊断的比较研究 被引量:10

摘要 目的:探讨64层螺旋CT血管成像(CTA)诊断胚胎型大脑后动脉(FTP)的准确性及其临床价值。方法:回顾性分析62例同时做了64层螺旋CTA和DSA脑血管检查患者的影像学资料,将CTA诊断FTP的结果与DSA检查结果进行比较,分析CTA诊断FTP的准确性。结果:62例中,DSA诊断FTP10例,CTA诊断FTP13例,CTA诊断FTP的敏感性为100%,特异性为96.40%,准确性为96.77%。经Kappa一致性检验,CTA与DSA诊断FTA的结果高度一致(P<0.001)。结论:64层螺旋CTA能够无创性准确判断FTP的存在,对筛选FTP患者具有重要意义,能为脑血管疾病的诊断与治疗提供重要信息。 Objective: To assess the accuracy and clinical value of 64-slice spiral CT angiography (CTA) on diagnosing of fetal origin of the posterior cerebral artery (FTP). Methods: Six-two patients in whom an intracranial 64-slice CTA and digital subtraction angiography (DSA) had been performed were analyzed retrospectively. This study compared the accuracy of CTA with DSA in the diagnosis of FTP. Results: In the 62 patients, CTA diagnosed 10 cases as FTP and DSA revealed that of 13 cases. Sensitivity, specificity and accuracy of CTA in the assessment of FTP could be estimated at 100 %, 96.40 % and 96.77 %, respectively. The results of CTA and DSA diagnosed FTP were highly coincident using the Kappa consistency checking. Conclusions: 64-slice CTA is very accurate in the assessment of the presence of FTP noninvasivly and may play an important role in screening FTP patients. It can supply important information for the diagnosis and therapy of cerebral vascular disease.
