严重腹部电烧伤临床分析

Clinical analysis of severe electrical abdominal injury

目的探讨严重电烧伤全层腹壁及腹腔内脏器损伤的诊治方法。方法收集1992年3月至2006年4月5例严重腹部高压电烧伤采用皮瓣转移或植皮术进行修复的临床资料,对其进行同顾性分析。结果采用皮瓣转移或植皮术进行修复的5例患者,4例皮瓣全部成活,其中3例皮瓣Ⅰ期愈合,1例皮瓣远端缺血坏死;1例术后3小时病情恶化抢救无效死亡。结论严重腹部电烧伤治疗的关键在于把握剖腹探查的时机和选择恰当的皮瓣。

Objective To explore the methods for the diagnosis and management of electrical ab- dominal injury involving the whole abdominal wall and celiac internal organs. Methods Five cases with severe electrical abdominal injury during March 1992 to April 2006 were enrolled in the study. And their clinical data were collected and analyzed. Results There were 4 out of 5 cases inflicted by the necrosis of celiac internal organs, Operations were performed in all 5 cases among which 4 received laparotomy ventrotomy. The necrotic organs were removed. And the burn wound in the trunk and extremities were covered after the recovery of abdominal internal organs. Grafting of skin and skin flap were employed in all the 5 cases to cover the abdominal defect wounds. Four out of the 5 cases were cured with one died finally. Conclusion Laparotomy ventrotomy should be performed immediately after the establishment of the diagnosis of severe electrical abdominal injury. Grafting of skin and skin flap could be employed for the repairing of the wounds.
