卵巢上皮肿瘤中ERα PR及E-Cadherin的表达及意义

The expression and significance of ERα,PR and E-cadherin in epithelial ovarian carcinoma

摘要 目的探讨卵巢上皮肿瘤各亚型中雌激素受体α(ERα)及靶蛋白孕激素受体(PR)、E-钙黏着蛋白(E-cad-herin)间表达的相关性及临床意义。方法对解放军总医院2001年1月至2006年1月经病理检查证实的67例卵巢癌标本[5例浆液性交界性腺癌、46例浆液性腺癌(SAC)、6例子宫内膜样腺癌(MAC)、5例透明细胞癌(CCA)及5例其它恶性肿瘤亚型],采用免疫组化SP法检测其ERα、PR、E-cadherin的表达情况,结合临床病理特征分析3种蛋白间表达的关系。结果(1)浆液性交界性卵巢癌、SAC、MAC、CCA中ERα表达阳性率分别为:40.0%、76.08%、66.67%、0;PR表达阳性率分别为:60.00%、82.61%、66.67%、0;E-cadherin表达阳性率分别为:100.00%、95.65%、100.00%、100.00%;SAC与CCA间ERα、PR的表达阳性率差异有统计学意义(P<0.05)。(2)E-cadherin的表达与SAC临床分期存在相关性(r=0.3610,P<0.05)。(3)SAC中,各临床病理特征(临床分期、病理学分级、年龄、淋巴转移)进行内部比较ERα、PR、E-cadherin的表达阳性率无明显差异。(4)浆液性卵巢癌中ERα、PR、E-cadherin的表达未检测出相关性(P>0.05)。结论ERα可能成为临床区分CCA和SAC的一项重要特征;E-cadherin可能在SAC的癌变过程中起重要作用,对临床诊断及预后评估具有一定的指导意义。 Objective To study the correlation between estrogen receptora (ERa), target protein progesterone receptor (PR) and E - cadherin in epithelial ovarian carcinoma and their clinical significance. Methods From Jan. 2001 to Jan. 2006,inlmunohistochemical staining method was used to examine the expression of ERa, PR, E - cadherin in 5 cases of serous borderline cancer, 46 cases of serous adenocarcinoma (SAC), 6 cases of endornetriod adenocarcinoma(EAC), 5 cases of clear cell adenocarcinoma(CCA) and 5 cases of other types of ovarian carcinoma in PLA General Hospital to analyze the correlation among them. Results ( 1 ) In ovarian serous borderline cancer, SAC, EAC and CCA, the positive rate of ERa was 40.0%, 76. 08%, 66. 67% and 0 respectively. The expression of PR was 60. 00%, 82.61%, 66. 67% and 0, and E - cadherin was 100. 00% and 95.65% , 100. 00% and 100. 00% respectively. There was significant difference in ERa and PR expression between SAC and CCA (P 〈 0. 05 ). (2) In SACs, a subtype of malignant ovarian cancer, the expression of E - cadherin was correlated to the clinical stage, r = 0. 3610, P 〈 0. 05. (3) In SACs, the positive rate of ERa did not make difference in early or advanced stage, well or poor differentiated, young(≤50) or elderly ( 〉 50 ), and presence or absence of lymphatic metastasis, which was the same with PR and E - eadherin. (4) In serous ovarian cancer, no correlation was found among ERa, PR and E - cadherin. Conclusion ERa and PR can be biological characteristics that distinguish CCA from SAC ; E - cadherin may play an important role in normal ovary transfeming to lnalignant cancer and can be a good index for clinical diagnosis and prognosis.
