摘要
目的探讨环氧化酶2(COX-2)、血管内皮生长因子(VEGF)在乳腺癌组织中的表达及其对预后的临床意义。方法收集我院1999-2006年有完整存档蜡块资料的117例乳腺癌蜡块以随机分配的原则组成组织芯片,并对其进行免疫组织化学染色,检测COX-2、VEGF在乳腺癌中的表达,并且对二者的表达情况与疾病的病理生物学特点、患者的生存时间进行分析,以判断其对预后的意义。结果COX-2阳性表达55.6%,COX-2在肿瘤中的表达与肿瘤临床分期(r=0.238、P=0.01)、淋巴结转移(r=0.326、P<0.01)、表皮生长因子2(HER-2)(r=0.218、P=0.025)呈正相关。与雌激素受体(ER)(r=-0.205、P=0.026)状态呈负相关。VEGF同样在乳腺癌组织中有高表达为66.7%,且与COX-2的表达在统计学上具有正相关(r=0.608、P<0.01)。COX-2和VEGF表达水平均与患者生存时间有密切关系。结论COX-2、VEGF在乳腺癌中均有高表达,且均与乳腺癌预后有关,可以作为乳腺癌预后的重要指标。
Objective To investigate the expression of cyclooxygenase-2 (COX-2) and vascular endothelial growth factor (VEGF) in the cancer tissue of patients with breast cancer and their clinical significance to prognosis. Methods 117 paraffin blocks of breast cancer had been collected in our hospital from 1999 to 2006, which were miemarrayed randomly in tissue chips and stained immunohistochemieally. The expression of COX-2 and VEGF in breast cancer were detected,and the relationships among their expression,the pathobiological behavior, survival time of breast cancer were analyzed to evaluate their prognostic significance. Results The expression of COX-2 was positive in 55.6%, which were positively correlated with some biological behaviors of breast cancer (TNM stage,lymph node metastasis and HER-2 and ER state ) ( P = 0.01, r = 0.238, P 〈 0.001, r = 0.326, P = 0.025, r = 0.218, P = 0.026, r = -0.205, respectively ), and was negatively cormlated with ER. High expression of VEGF was also found in breast cancer tissues (66.7%) ,which had a positive correlation with COX-2 expression in statistics (P 〈 0.001 ,r = 0.608). The survival time of patients with positive COX-2 expression and VEGF expression was significantly shorter than that without the expression. Conclusion The expressions of COX-2 and VEGF are high in breast cancer tissues and are associated with the prognosis of breast cancer, which can be used asan important prognostic index of breast cancer.
