小隐静脉-腓肠神经营养血管筋膜皮瓣急诊修复小腿足踝部软组织缺损 被引量:2

摘要 【目的】探讨小隐静脉-腓肠神经营养血管筋膜皮瓣急诊一期修复小腿足踝部软组织缺损的方法及疗效。【方法】38例小腿足踝部软组织缺损的急诊病人予以严格彻底的清创,再采用小隐静脉-腓肠神经营养血管筋膜皮瓣,急诊一期修复创面。术中采用低轴点、远端结扎小隐静脉等方法扩大皮瓣切取面积、减轻皮瓣肿胀。【结果】38例全部成活,无软组织感染。其中5例远端带蒂皮瓣边缘部分坏死,经常规换药后脱痂愈合。平均随访8个月,皮肤质地良好,有部分感觉恢复。【结论】采用小隐静脉-腓肠神经营养血管筋膜皮瓣,急诊一期修复小腿足踝部软组织缺损效果优良。采用低轴点、远端结扎小隐静脉等方法可明显改善皮瓣肿胀。
