摘要 目的通过监测患者的肌酐清除率,实现个体化给药,为临床提供优质的药学服务。方法监测使用左氧氟沙星(左克)注射液、哌拉西林/他唑巴坦(海他欣)注射液、头孢哌酮/舒巴坦(可倍)注射液3种药物的老年患者(年龄>55岁)血清肌酐浓度,计算肌酐清除率。监测患者的用药情况。结果在老年患者中普遍存在生理性肾功能减退现象。左氧氟沙星注射液在老年患者中存在大剂量使用现象,而患者肌酐清除率不符合用量要求的约占1/3。结论为了达到理想的治疗效果,通过监测老年患者肌酐清除率,对实现个体化给药,减少不良反应的发生具有重要意义。 Objective To implement the individualized drug administration by monitoring the creatinine clearance rate of patients, and to provide high quality service for clinical pharmacy. Methods The creatinine clearance rate of aged patients ( 〉55 ) was calculated using one of the three injections (levofloxacin, piperacillin/tazobactam and cefoperazone./culbatam), by monitoring serum ereatinine concentration in renal function measurement. The drug administration for those patients was monitored. Results The physiological decreased renal functioning was popular among aged patients. The high-dose using of levofloxacin injection to ages patients was found, and in about 1/3 of these patients, the creatinine clearance rate did not meet the request of dosage. Conclusion Monitoring the creatinine clearance rate of aged patients is important in achieving ideal curative effect, implementing individualized drug administration, and decreasing adverse effects.
