低钾性周期性瘫痪46例临床分析

Clinical Analysis on 46 Patients with Hypokalemic Periodic Paralysis

目的探讨低钾性周期性瘫痪的临床特点,提高诊治水平。方法对46例低钾性周期性瘫痪的临床资料进行回顾性分析。结果多数发病有诱因,睡眠及晨醒时发病;典型表现是反复发作的以肢体近端为主的四肢驰缓性瘫痪,严重者呼吸肌无力,有血清钾降低;所有病例经口服、静脉补钾后症状均获缓解。部分患者并不符合上述典型临床表现:如瘫痪不对称、腱反射亢进等,治疗过程中出现一过性血清钾升高。结论认识低钾性周期性瘫痪的不典型临床表现对早期诊断、及时治疗具有一定意义。

Objective To investigate the clinical characteristics of hypokalemic periodic paralysis（HOPP） and explore the diagnosis and treatment of HOPP. Methods The clinical data of 46 cases with HOPP were analyzed and summarized retrospectively. Results There was an inducement among mostly eases, and the symptoms occurred mainly in insomnia or early awakening. The classical appearance was sluggish myoasthenia of four limbs and hypopotassemia. Serious HOPP experienced respiratory paralysis. The symptoms of all cases relieved completely after intravenous or oral supplement of potassium. Some patients did not show the above typical clinical manifestations ; the transient hyperkalemia could be observed in the course of treatment. Conclusions The understanding to untypical clinical features of hypokalemie periodic paralysis is helpful to the early diagnosis and timely treatment.
