前列腺素E_1对老年肺心病患者氧代动力学的影响

Effect of PGE 1 on the oxygen kinetics in elderly patients with cor pulmonale

目的评估前列腺素Ｅ１（ＰＧＥ１）对老年肺心病患者的氧代动力学、血流动力学和血液流变学的作用。方法对１２例老年肺心病患者静脉滴注ＰＧＥ１（４０μｇ·ｋｇ－１／ｍｉｎ），静滴前、后测定氧输送、氧消耗、氧摄取率、血液动力学参数及血液流变学。结果ＰＧＥ１可增加老年肺心病患者心脏指数、氧输送、氧消耗和氧摄取率；降低肺及体循环的血管阻力指数；改善血粘度。结论ＰＧＥ１通过降低肺血管阻力、改善肺内血流分布和增加心输出量，从而改善了氧供给和血液流变性，抑制了由于乏氧性缺氧而代偿性增加血红蛋白的负面效应——氧摄取缺陷。

Objective To evaluate the influence of PGE 1 on the oxygen kinetics,hemodynamics and blood rheology in 12 elderly cor pulmonale patients.Methods PGE 1 was dripped intravenously at a rate of 40 μg·kg -1 /min.The oxygen kinetics,hemodynamics and blood rheology parameters were observed before and after the administration of PGE 1 Results PGE 1 increased the cardiac index, oxygen transport, oxygen consumption and oxygen up take,decreased the hemo resistance index of the pulmonary and systemic circulation,and improved the blood viscosity. Conclusions PGE 1 could improve the oxygen supply and blood rheology and inhibit the negative influence of the compensatory increase in hemoglobin due to anoxic oxygen deficiency,through its effect of lowering the pulmonary vascular resistance,improving the intrapulmonary blood distribution and increasing the cardiac output.
