血小板参数在心绞痛病人中的变化

摘要 目的观察血小板计数(PC)、血小板分布宽度(PDW)、平均血小板体积(MPV)、大型血小板比率(P-LCR)、血小板压积(PCT)等血小板参数在不稳定性心绞痛(UA)和稳定性心绞痛(SA)病人中的变化。方法选取2006年1月至2007年8月,以经过冠状动脉造影证实的185例SA、98例UA病人为研究对象,32例冠状动脉造影正常的胸痛患者作为正常对照组,分别测定PC、PDW、MPV、P-LCR、PCT等血小板参数并进行比较。结果PC在SA和对照组中无显著性差异,UA组低于SA组和对照组。UA组的PDW、MPV、P-LCR高于SA组和对照组,SA组高于对照组。PCT在3组之间无显著性差异。结论血小板参数分析有助于UA和SA的诊断和鉴别诊断。 Objective To observe the changing rules of platelet count (PC), platelet distribution width (PDW), mean platelet volume(MPV), Platelet-large cell ratio(P-LCR), Platerit (PCT) in unstable angina compared to stable angina. Methods From January 2006 to August 2007, there were 185 patients defined clinically as stable angina, 98 unstable angina and 32 non-cardiac chest pain for diagnostic coronary angiography. The patients with a normal coronary angiogram and chest pain were classified as having non-cardiac chest pain and were defined as a control group. PC, PDW, MPV, P-LCR and PCT were analyzed with blood cell counter for every patient. Results Patients with stable angina similarly had no significant difference in platelet count compared to the control group but did have a significantly increased PDW, MPV and P-LCR . In contrast, patients with unstable angina had a decreased platelet count compared to either stable angina or the control group. Furthermore, PDW, MPV and P-LCR were greater in unstable angina. Conclusion Larger platelets are haemostatically more active and are a risk factor for developing coronary thrombosis. Patients with larger platelets can easily be identified during routine haematological analysis and can possibly benefit from preventive treatment. Thus, platelet parameters are an important, simple, effortless, and cost effective tool that should be used and explored.
