59例梗阻性黄疸螺旋CT征象分析

Analysis of Spiral CT Manifestation in 59 Cases of Obstructive Jaundice

目的回顾性分析梗阻性黄疸的螺旋CT征象,以提高诊断准确率。方法收集59例梗阻性黄疸患者的影像学资料,结合病理进行分析。结果在所有梗阻因素中恶性黄疸占91.5%,CT表现为肝内胆管扩张、胆管呈"软藤征"、"鼠尾征",胆总管多突然"截断",梗阻部可见低密度肿块、结节,延迟强化明显,良性梗阻胆总管多渐进性狭窄。结论螺旋CT是临床诊断梗阻性黄疸的有效手段,恶性肿瘤是导致胆道梗阻的主要因素。

Objective Spiral CT findings of obstructive jaundice were analyzed retrospectively so as to improve the accuracy of diagnosis. Methods The medical image materials on 59 cases of obstructive jaundice were collected and analyzed with pathology Method. Results Malignant jaundice took up 91.5% among all the obstructive factors. CT scan showed intrahepatic bile duct dilation, the intrahepatic duct showed as soft rattan sign or mouse tail sign, the common bile duct was mostly manifested interruption, low-density mass and nodules were usually seen at the region of obstruction, the delayed enhancement was obvious, the benign obstructive common bile duct was manifested as gradual stenosis. Conclusion Spiral CT proves to be an effective clinical diagnosis for obstructive jaundice ; malignant tumor is the leading factor resulting in biliary tract block.
