摘要 目的验证Stanford B型主动脉夹层形成的条件,探讨血流因素和管壁因素的相关性。方法12只实验动犬随机分为三组,每组4只,分别进行血流因素、管壁因素及血流、管壁双因素的胸主动脉夹层动物模型建立,分别用单因素敏感性分析及双因素敏感性分析血流和管壁的变化与Stanford B型主动脉夹层形成的相关性。结果血压升高及中膜破坏为胸主动脉夹层形成的敏感因素。经统计学处理表明,在胸主动脉夹层形成的综合因素里中膜的病变贡献率最大。结论管壁因素和血流因素都是胸主动脉夹层形成的必需条件。其中管壁因素是基础因素,血流因素是始动因素。 Objective To confirm the pathogenesis of Stanford type B aortic dissection (AD) and to find the relativity between the formational factor of aortic wall and the blood flow factor in Stanford type B aortic dissection. Methods Twelve dogs were randomly divided into 3 groups ( n = 4 in each group) and 3 types of animal models were established. The relationship between the changes of aortic wall and the blood flow with Stanford BAD was analyzed. Results The high blood pressure (HBP) and the damage to the tunlca media were the two sensitive factors. Statistical analysis showed the membrane lesions did the greatest contribution in the procedure of AD. Conclusion Factors of wall and blood flow are the necessary conditions of AD. In which, the wall factor is the basic one and the blood flow factor is the motivating factor.
