摘要 目的探讨脑神经胶质瘤术后复发和播散的螺旋CT表现与组织学类型。方法回顾性分析33例脑胶质瘤术后的螺旋CT资料,总结脑胶质瘤复发和播散的类型及复发时间分布特点,并进行统计学分析。结果33例中有20例脑胶质瘤术后复发和播散,其中包括10例胶质母细胞瘤,4例星形细胞瘤,3例髓母细胞瘤,2例少支胶质细胞瘤,1例胶质肉瘤。复发和播散常发生在术后2个月至1a内。结论脑胶质瘤术后复发通常起源于手术切缘周围2cm范围内生长。播散主要为脑脊液种植播散。螺旋CT扫描是脑胶质瘤术后复发和播散随诊观察的有效手段。 Objective To discuss the spiral CT findings and histopathologic types of postoperative brain glioma recurrence and dissemination(PBGRD). Methods Thirty - three patients with surgically and pathologically proved brain glioma whose CT data were analyzed retrospectively, simultaneously, the histopathologic types and the time intervals of PBGRDs were summarized via statistic analysis. Results Among 33 cases, PBGRDs were found in 20 patients who comprised 10 glioblastomas, 4 astrocytomas, 3 medullobrastomas, 2 oligo- dendrogliomas, and 1 gliosarcoma. The time interval between the diagnosis of the primary tumor and the diagnosis of recurrent tumor was 2 months - 1 year. Conclusion Postoperative brain glioma recurrence commonly occurs with 2 cm of incision periphery, while the dissemination mainly presents as cerebrospinal fluid plant - dissemination. Spiral CT scanning is an effective means for follow - up observation of PBGRDs.
