数字化多层螺旋CT对骨关节创伤的评价 被引量:4

摘要 目的探讨数字化多层螺旋cT对骨关节创伤的应用价值。方法对66例骨关节创伤患者行64层螺旋CT扫描,并进行二维多平面重建;23例患者行三维容积再现,对3例疑血管病变的患者行64层螺旋CT血管成像检查,对不同重建方法进行分析。结果64层螺旋CT扫描成像对骨关节创伤显示清楚,诊断正确率为100%,二维多平面重建有助于骨折细节和错位的显示及软组织损伤的显示,三维容积再现有助于骨折和局部复杂的解剖结构直观的立体显示。结论数字化多层螺旋CT有助于创伤的正确分型和临床治疗方案的选择,可以对骨关节创伤做出正确的术前诊断并评价其疗效。 Objective To evaluate the diagnostic value of digital multi-slice spiral CT (MSCT) for the trauma of bone and joint. Methods Sixty-six patients with trauma of bone and joint underwent 64-slice spiral CT scanning and 3 patients with angiopathy underwent 64-slice spiral CT angiography before 2D multiple planar reconstruction (MPR). 3D volume rendering (VR) was performed for 23 patients, and maximum intensity projection (MIP) and curved planar reformation (CPR) for the 3 patients, The results of different reconstructions were analyzed. Results The traumas of hone and joint were well demonstrated on 64-slice spiral CT images so that the rate of correct diagnosis was 100%. 2D multiple planar reconstruction (MPR) was helpful in observing the apposition and alignment of the fractures and the soft tissue injury, 3D volume rendering (VR) directly displayed the complex anatomic details and stereoscopic visualization of the fractures. Conclusion Since digital multi-slice spiral CT is helpful in correctly classifying traumas of bone and joint and clarifying complexity of fractures, it can help surgeons make correct preoperative diagnosis and thus select an appropriate treatment protocol as well as can evaluate the outcome of treatment
