不同手术方法治疗乳糜尿的远期疗效对比观察(附66例报告)

A case-control observation of the treatment of chyluria with various ways

为探讨乳糜尿的较佳治疗方法，采用三种不同的手术方法共治疗乳糜尿66例，其中采用肾蒂淋巴管结扎术33例，14例同时施行肾纤维膜剥脱并翻转包裹肾蒂（简称A组）；采用腹股沟淋巴结大隐静脉吻合术16例（简称B组）；采用肾蒂淋巴管结扎同时行腹股沟淋巴结大隐静脉吻合术17例（简称C组）。结果53例获得随访，随访时间为2．2～14．0年。随访2～4年，乳糜尿消失率3组分别为76．0％、57．1％和92．3％；随访5年以上，治愈率3组分别为60．0％、50．0％和85．7％。认为肾蒂淋巴管结扎同时行腹股沟淋巴结大隐静脉吻合术的远期疗效明显优于其他术式。

The surgical treatment of 66 cases of chyluria through three ways was reported.There were 33 cases used with ligature of renal pldicle entrering lymphatic vessel-among them there were 14 cases treated by denodation of renal fibrous membrance. resup1nation and encapsu1ation (short in A group ).There were 16 cases used with anastomosis between inguinal lymph nodes and great saphenous (short in B group). 17 cases were treated by ligature of renal pldicle lymph node and anastomosis between inguinal lymph nodes and great saphenous (short in C group). Among them, 53 cases had been followed up 2. 2～14. 0 years. In 2～4 years the disappearing rate of Chyluria is 76. 0%, 57. 1% and 92. 3%. The cure rate is 60.0%, 50.0% and 85. 7%,Among these groups,the treating effect in the last group is higher than the first two groups.
