埃索美拉唑在治疗幽门螺杆菌感染的消化性溃疡中的地位

Treating Hp-related digestive ulcer with esomeprazole

目的比较埃索美拉唑三联与奥美拉唑三联疗法治疗幽门螺杆菌(Hp)阳性消化性溃疡临床疗效。方法将60例经内镜诊断并检测证实Hp阳性的消化性溃疡患者随机分为两组。埃索美拉唑组(30例):埃索美托唑20 mg+阿莫西林1 g+克拉霉素500 mg,每日2次,共7 d;奥美拉唑组(30例):奥美拉唑20 mg+阿莫西林1 g+克拉霉素500 mg,每日2次,共7 d。疗程结束4周后复查胃镜并检测Hp,观察腹痛缓解率、溃疡愈合率、Hp根除率及用药后的不良反应等。结果埃索美托唑组和奥美拉唑组溃疡愈合率分别为90.1%和83.9%,Hp根除率分别为94.2%和83.7%,差异有统计学意义(P>0.05)。两组用药后不良反应少,有较好的安全性。结论埃索美托唑三联疗法治疗Hp阳性的消化性溃疡安全有效,优于奥美拉唑三联疗法。

Objective To investigate the effect of esomeprazole in the treatment of digestive ulcer with helicobacter pylori（Hp） infection. Methods A total of 60 patients with digestive ulcer and Hp infection were randomly divided into group A （ 30 cases） treated with esomeprazole （20mg）, amoxicillin （1000mg） and clarithromycin （500 mg） twice daily and group B（30 cases） treated with omeprazole（20mg）, amoxiciUin（1000mg） and clari thromycin （500mg） twice daily. The treatment lasted for 7 days,after which abdominal pain relief rate,ulcer healing rate and eradication rate of Hp as well as adverse effects of the medication were evaluated and compared. Results Ulcer healing rate and Hp eradication rate were 90.1% and 94.2 % in group A and 83.9 % and 83.7 % in group B,showing significant statistical diferences between the two groups（P 〈 0.05）. The adverse effects of the medication were rare in both groups without significant differences. Conclusion Esomeprazole-based triple therapy might effectively erad- icate Hp infection and promote ulcer healing with good tolerance.
