摘要
目的了解河北省成年居民(18~69岁)中高血压患者危险因素状况。方法采用多阶段分层抽样的方法确定样本,统一培训调查员,采用统一的调查表进行问卷、体格和血压测量、血样检测等方法。高血压分为三类亚型,单纯收缩期高血压(ISH);单纯舒张期高血压(IDH);收缩期舒张期高血压(SDH)。结果调查的4200成年居民中有1663例高血压患者,ISH、IDH、SDH分别占26.4%、23.2%、50.5%;构成比单因素分析显示:ISH在55~64岁组(33.0%)、女性(62.2%)、小学文化(34.6%);IDH在35~44岁组(39.7%)、男性(50.1%)、初中文化(44.9%);SDH在45~54岁组(36.6%)、女性(55.4%)、初中小学文化(63.3%)占的比例最高。多因素逐步聚类分析显示:三种高血压患者危险因素有不同的聚集性。结论河北省成年居民中高血压患者以SDH为主,我们对不同的高血压患者应进行不同健康促进方式。
Objective To analyze the risk factors of hypertensionamong adult residents ( 18-69yr) in Hebei province. Methods The adultresidents were selected by multistage stratified sampling, and were investigated with unified questionnaires, body measurement, blood pressure measurement, blood sample analysisby unified-trained investigators. Hypertension were divided into the subtypes, isolated systolic hypertension (ISH), isolated diastolic hypertension (IDH) and combined systolic and diastolic hypertension (SDH). Results There were 1 663 hypertensive patients among 4 200 adults residents. The composition ratios of ISH, IDH and SDH were 26.4%, 23.2% and 50.5% respectively. The analysis of single factor indicated that the highest composition ratioof ISH was in 55-64 age group (33.0%), female accounted for 62.2%, primary school in education accounted for 34.6% ; which of IDH was in 35-45 age group (39.7%), male (50.1% ), junior high schoolin education(44.9% ) ;which Of SDH was in 45 - 54 age group (36.6%), female (55.4%), primary and junior high school in education (63.3%). The multi-factors cluster analysis indicated that the hypertensive patients in three subtypes had the different cluster in the risk factors. Conclusion SDH is the main subtype among adult hypertension in Hebei province. It indicated that the varieties ways of health promotion should be taken.
