超声造影对睾丸扭转的诊断价值 被引量:14

Clinical value of contrast-enhanced ultrasonography in diagnosis of testicular torsion

摘要 目的探讨超声造影对睾丸扭转的诊断价值。方法对24例单侧睾丸扭转患者首先进行二维超声及彩色多普勒超声检查,再用Sono Vue经肘静脉团注法进行超声造影检查,并对患侧睾丸与正常侧睾丸的造影剂增强情况进行对比定量分析。所有病例均经手术病理证实。结果24例睾丸扭转患者中19例为不全扭转,其达峰时间及到达时间与正常侧比较差异有统计学意义(P〈0.001);峰值强度及增强强度与正常侧比较差异有统计学意义(P〈0.05)。5例完全扭转的睾丸超声造影增强不明显。结论超声造影诊断睾丸扭转具有较高的准确性,是彩色多普勒超声诊断的进一步补充。 Objective To investigate the clinical value of contrast-enhanced ultrasonography (CEUS) in diagnosing the testicular torsion. Methods Twenty-four suspected unilateral torsional testicles were examined by B-mode ultrasound and colour Doppler flow imaging(CDFI), then CEUS was performed via intravenous bolus injection of SonoVue. Which findings were compared with surgery. Results Arrive time (AT) and time to peak (TTP) in 19 cases of incomplete torsional testicles were delayed than those of normal contralateral testicles(P〈.001);Peak intensity (PI) and enhancement intensity (El) were also lower than those of normal contralateral testicles(P〈0.05). There were no significant enhancement in 5 cases of complete torsional testicles. Conclusions CEUS has high sensitivity and specificity in diagnosing testicular torsion, which can be considered as a further complement of CDFI in diagnosing testicular torsion.
