摘要 目的评估急性缺血性脑卒中时胃屏障功能的变化,并研究蔗糖通透性试验作为评价胃屏障功能指标的有效性。方法20只犬随机分为实验组和假手术组,每组10只。将实验组犬制成缺血性脑卒中模型后,经胃管注入含50g蔗糖的生理盐水300ml,分别于5和24小时检测尿蔗糖含量。脑栓塞后24小时处死犬进行全胃大体病理学分级和组织学积分评估,并与假手术组进行比较。把蔗糖通透性试验和大体病理学分级及组织学积分结果做相关性分析,以判断蔗糖通透性试验的有效性。结果实验组在脑栓塞后5小时[(406.19±188.44)mg]和24小时的尿蔗糖含量[(418.78±178.67)mg]均显著高于假手术组[(78.91±61.79)mg和(166.21±144.45)mg](P均<0.05)。实验组和假手术组的胃大体病理学分级(1.70±0.95vs.0.30±0.48,P<0.05)和组织学积分(5.20±1.99vs.2.40±1.51,P<0.05)均具有显著性差异。尿蔗糖含量和胃大体病理学分级和组织学积分均呈正相关(r=0.89,P<0.05;r=0.83,P<0.05)。结论急性缺血性脑卒中时胃屏障功能快速、显著破坏。蔗糖通透性试验可靠易行,是评价胃屏障功能的一种很有前景的方法。 Objective To evaluate the change of gastric barrier function in the stress of ischemic stroke, and to assess the value of sucrose permeability in the evaluation of gastric barrier function. Methods Twenty dogs were randomly divided into study group ( n = 10) and sham group ( n = 10 ). The dogs in the study group were made ischemic stroke and fed with 50 g sucrose within 300 ml normal saline by gastric tube, and the 5-hour and 24- hour urine sucrose excretion were measured. The dogs were sacrificed 24 hours after infarction and gastric gross classification and histologic scoring were performed. All the results were compared with those of 10 dogs in the sham group. Correlation analysis was performed for the results of sucrose permeability test, gastric gross classification, and histologic scoring. Results The sucrose permeability 5 hours [ (406. 19 ± 188.44) mg] and 24 hours [ (418.78 ± 178.67) mg] after ischemic stroke in the study group were significantly different from the results in the sham group [ (78.91 ±61.79) mg and (166.21 ±144.45) mg, respectively] (both P 〈 0. 05). The gastric gross classification ( 1.70 ± 0.95 vs. 0. 30 ± 0. 48, P 〈 0.05 ) and histological score (5.20 ± 1.99 vs. 2.40 ± 1.51, P 〈 0.05 ) in the study group were all significantly different from the results in the sham group. The resuh of the sucrose permeability was positively correlated with that of the gastric gross classification ( r = 0.89, P 〈 0.05 ) and histologic scoring ( r = 0.83, P 〈 0.05 ). Conclusion The gastric barrier is rapidly and severely damaged during ischemic stroke, and the measurement of sucrose permeability is a simple and reliable method to assess gastric barrier function.
