颞部皮瓣在Treacher-Collin′s综合征的应用

Application of temporal flap in Treacher-Collin's syndrome

目的探讨颞部皮瓣在Treacher-Collin′s综合征中修复眶缘及颧部畸形的效果。方法设计邻近的颞区皮瓣,结合硅胶或MEDPOR材料填充,修复眶周及颧部的凹陷畸形,行颧眶部再造术。结果本组共5例患者,术后随访6～24个月,移植皮瓣全部成活,供区瘢痕不明显,睑裂无变形,皮瓣色泽与面部皮肤接近,颧部饱满,效果满意。结论颧眶部再造是Treacher-Collin′s综合征治疗的基础和最重要部分。邻近的颞部皮瓣血供可靠,转移方便,色泽协调,供区隐蔽,结合硅胶或MEDPOR材料填充是修复Treacher-Collin′s综合征中眶周及颧部畸形的一种有效方法。

Objective To evaluate the effects of use of temporal flaps for repairing the deformities of the orbital margin and pars zygomatica in Treacher- Collin＇s syndrome. Methods Reconstraction of the temporal region was performed by design of a flap around the temporal region combined with filling with silica gel or Medpor prosthesis for repair of the depressed deformity in orbital margin and pars zygomatica. Results After 6 - 24 months follow- up, all of the flaps survived with scar will concealed at the donor site. Unchanged lid fissure, similar color of the flap and face and well - filled temporal region. The results were satisfactory. Conclusion Malar and orbital margin reconstruction is the most important part of treatment of patients with Treacher, Collin＇s syndrome. Our method was effective in treating such patients, and has such advantages as more reliable blood supply, easy flap transfer, flap color similar to the face, and well concealed scar at the donor site.
