荧光皮肤血流仪在兔皮瓣血循环判断中的初步应用 被引量:3

摘要 目的探索荧光皮肤血流仪在兔皮瓣血循环判断中的应用规律。方法以新西兰兔为实验动物,于背部设计一16cm×4cm任意型皮瓣,切取、剥离皮瓣后原位缝回。术后荧光素钠4mg/kg耳缘静脉注射,20min后用荧光皮肤血流仪依次测定皮瓣不同部位荧光强度,计算荧光强度指数(DFI)。1周后观察皮瓣成活情况,测算皮瓣成活与坏死面积,分析DFI数值与组织活力间的关系。结果1周后皮瓣远端发生坏死,近端成活良好,平均坏死面积54%,平均成活面积46%;成活区域DFI平均值为66%,坏死区域DFI平均值为16%,二者比较有统计学意义(n=7,P<0.01)。统计分析显示,DFI≤25%时,组织可能发生坏死。结论荧光皮肤血流仪DFI与兔皮瓣血循环灌注有良好的对应关系,通过荧光皮肤血流仪可以对皮瓣组织活力进行较为准确的判断。 Objective To explore the application of a dermofluorometer in assessment of blood circulation in rabbit skin flaps. Methods Seven New - Zealand rabbits were selected as experimental animals, and 7 random skin flaps of 16 cm× 4 cm on the hacks of these rabbits were elevated and sutured back into the same place. After surgery, 4 mg/kg fluoreseein sodium was administrated by intravenous injection, and fluorescence intensity in different areas of these skin flaps was evaluated by dermofluorometer 20 rain after administration of this agent, and DFI was calculated. Seven days later, the rate of flap survival and extent of necrotic skin these flaps were calculated, and then the relation between DFI and vitality of flap tissue was analyzed. Results One week later, distal parts of skin flaps were necrotic, while proximal parts were viable. The average extent of necrotic areas was 54 %, while that of surviving areas was 46 %. The average value of DFI in surviving areas was 66 %, while that in necrotic areas was 16 %, with a statistical difference of P ≤ 0.01 ( n = 7). Statistical analysis showed that tissues could become necrotize when DFI Values were ≤25%. Conclusion DFI measured by use of a dermofluorometer showed good correspondence to the blood perfusion of rabbit skin flaps. The dermofluorometer was able to accurately assess the vitality of the rabbit skin flaps.
