食管癌c-erbB-2原癌基因的表达及意义

A Study on the Expression of c erbB 2 Oncogene in Esophageal Cancer

目的为了探讨食管癌组织ｃ－ｅｒｂＢ－２的表达及其临床意义。方法采用ＳＰ免疫组织化学法对４１例食管癌标本进行ｃ－ｅｒｂＢ－２的检测。结果４１例食管癌中，ｃ－ｅｒｂＢ－２阳性表达率为５１．２％（２１／４１），其中高分化组ｃ－ｅｒｂＢ－２阳性率为３３．３％，中分化组为４２．１％，低分化组为７６．９％，低分化组和高分化组相比较，两组间有显著性差异（Ｐ＜０．０５）。在有淋巴结转移的食管癌中，ｃ－ｅｒｂＢ－２癌基因表达率为６５．５％（１９／２９），而无淋巴结转移者中的ｃ－ｅｒｂＢ－２癌基因阳性表达率为１６．７％（２／１２），二者相比有非常显著性差异（Ｐ＜０．０１）。１０例正常食管粘膜中ｃ－ｅｒｂＢ－２癌基因则均为阴性。结论ｃ－ｅｒｂＢ－２癌基因不仅是食管癌的一种新的标志物，而且还与食管癌的分化程度、淋巴结的转移有关，ｃ－ｅｒｂＢ－２的检测可成为食管癌诊断和判定预后有价值的指标。

Objective To explore the expression of c erbB 2 oncogene in esophageal carcinoma and its clinical significance. Methods c erbB 2 oncogene was assayed by immunohistochemistry in 41 esophageal cancer tissue. Results The positive rate of c erbB 2 oncogene expression was 51.2% (21/41) in esophageal cancer, 33.3% in well differentiated carcinoma, 42.1% in moderatedly differentiated cancer, and 76.9% in poorly differentiated cancer with an significant difference between poorly differentiated and well differentiated group ( P<0.05) . The positive rate was 65.5% in cases with lymph node metastasis, and 16.7% in cases with no lymph node metastasis with, a highly significant difference between the two groups ( P<0.01) . No c erbB 2 expression was observed in the normal esophageal mucosa. Conclusion These results suggest that there is a close relationship among the expressional level of c erbB 2 oncogene protein, tumor differentiation and lymph node metastasis. Determination of c erbB 2 oncogene can help predict the lymph node metastasis and evaluate the prognosis of patients with esophageal cancer.
