双侧胸腔同期一次胸腔镜手术33例

Vidco Assisted Thoracoscopic Operative Treatment for Bilateral Intrathoracic Diseases in 33 Cases

从１９９４年６月至１９９７年２月，为术前Ｘ线胸片或ＣＴ扫描提示双侧胸腔都有病变的病例，采用胸腔镜进行双侧胸腔同期手术的方法达到一次治愈的目的，共完成３３例。其中男２８例，女５例。术式包括双侧肺大切除术、肺楔形切除术、肺叶切除术、纵隔肿瘤切除术、肺减容术和胸膜瘘修补术等。该方法具有微创伤的特点，且暴露好。采用双侧胸腔同期内镜手术方法，一次治疗双侧胸腔的病变，总治疗支出减半。且近期效果满意，２年多随访疗效比传统方法稍优。作者认为，这种方法从治疗上和经济上均有较大的临床意义。

Aim:The advantages of video assised intrathoracic surgery in the respects of minimal trauma and fast postoperative recovery have been well appreciated by the thoracic surgeons in recent years.This paper is to illustrate the effects of bilateral video assisted thoracoscopic intrathoracic operations performed in succession.Method:From June 1994 to Feb.1997,bilateral video assisted thoracoscopic intrathoracic operations were performed on 33 patients,28 males and 5 females,with bilateral intrathoracic lesions.The mode of operations included bleb resection,wedge resection,lobectomy,resection of mediastinal tumor,lung volume reduction,pleural fistula,et al.The surgical exposure is much better than the approach through the mediastinal route.Results:All of the patients recovered well postoperatively.Follow up found satisfying long term results.Conclusion:This technique offers more benefits to the patients not only in minimizing surgical trauma and smooth postoperative course,but also in reducing operative expenses to half.
