替米沙坦对慢性心力衰竭患者心室重构影响的临床研究

Effects of telmisartan on reverse remodeling ventricle in patients with chronic congestive heart failure

摘要 目的观察替米沙坦治疗前后慢性充血性心衰患者心功能的变化,研究替米沙坦对心室重构的影响。方法52例慢性心衰患者在应用利尿剂、受体阻断剂、洋地黄治疗的基础上随机分为A组(n=24)和B组(n=28),A组给予替米沙坦40mg/d;B组给予贝那普利10mg/d口服。治疗前及治疗后6个月分别检查超声心动图测定心功能和心室重构指标。结果两组在治疗后6个月,NYHA分级较治疗前均有改善,差异有显著性(P<0.05);两治疗组间差异无显著性(P>0.05)。两组在治疗后6个月左室射血分数、左室收缩末内径、左室舒张末内径较治疗前均有改善,差异有显著性(P<0.05);两治疗组间差异无显著性(P>0.05)。结论替米沙坦和贝那普利具有相似的抗心室重构效应,替米沙坦的耐受性更好。 Objective To evaluate the effects of telmisartan on patients with chronic congestive heart failure (CHF). Fifty- two patients were randomly divided into group A consisted of 24 patients treated with telmisartan(40mg, po, qd) for 6 months and group B consisted of 28 patients given benasepril ( 10mg, po, qd) for 6 months. The changes of NYHA class, biochemistry, liver function, renel function and echecardiography were observed. Comparison of thickness of ventricle byultrasound was made before and after treatment. Results After 6 months of therapy, NYHA class, left ventricular end diastolic diameter (LVDD), left ventricular end systolic diameter (LVSD), and left ventricular ejection fraction (LVEF) of both groups were improved significantly (P 〈 0.05), without significant difference(P〉O,d5) betwecntwogroups. Conclusion Telmisartan is similar with benasepril in treatment of CHF, which is able to remodel the hypertrophied ventricle, and ameliorates the heart function.
