影像学检查在原发性甲状旁腺功能亢进症术前定位诊断中的作用 被引量:6

摘要 目的比较超声、CT、MRI、99mTc-MIBI(甲氧基异丁基异腈)在原发性甲状旁腺功能亢进症(PHPT)术前定位诊断中的价值。方法回顾性分析同济医院24例手术治疗的PHPT患者的临床资料。结果超声、CT、99mTc-MIBI双时相显像、MRI诊断符合率各为91%(20/22)、73%(11/15)、94%(15/16)、100%(4/4)。结论超声、CT、99mTc-MIBI双时相显像、MRI在原发性甲状旁腺功能亢进症术前定位诊断中具有重要的价值。超声和99mTc-MIBI核素显像应作为首选的术前定位检查方法,若条件允许可以行其它影像学检查。 Objective To evaluate the diagnostic value of US, CT, MRI and 99mTc MIBI in primary hyperparathyroidism (PHPT). Methods Data of 24 PHPT patients with diagnosis confirmed by postoperative pathology were retrospectively reviewed. Results The preoperative diagnosis in 20 out of 22 cases (91%) undergoing preoperative US of the neck with positive result was verified by operative findings. For CT, MRI and 99mTc-MIBI, the positive rate was 73%, 100% and 94% respectively. Conclusion US, CT, ^99mTc-MIBI and MRI play an important role in the preoperative loci localization in patients with PHPT. US and 99mTc-MIBI should be considered as the first choice for preoperative localization of tumors in patients with PHPT. US in combination with ^99mTc-MIBI is more sensitive and accurate for the localization of parathyroid gland.
