Objective To ensure rational, safe and effective drug use for discharged patients. Methods Pharmacists detailedly explained the contents, methods and things to pay attenion to discharged patients based on their prescriptions and drug instructions. Results Relevant knowledge of drugs was detailedly shown so as to avoid improper use of drugs, and to promote rational drug use. Conclusion Medication guidance can improve the drug therapy.
