食管癌变过程中Stat3和C-erbB-2基因表达的动态观察

The Dynamic Observation of the Expressions of Stat3 and C-erbB-2 Genes in the Process of Esophageal Carcinogenesis

目的动态观察Stat3和C-erbB-2基因在食管癌变过程中的表达,探讨其在食管癌变过程中的作用。方法本实验采用的标本为1998年和2000年内镜普查的食管活检组织标本。对1998年普查中诊断为异型增生的57例进行2年随访,40例仍为异型增生,定为未癌变组。17例进展为早期癌,为癌变组。对未癌变组和癌变组2次活检组织标本分别采用免疫组织化学S-P法进行Stat3和C-erbB-2蛋白的动态检测。结果Stat3和C-erbB-2在1998年未癌变组和癌变组中的表达差异无统计学意义(P>0.05);两者的表达在2000年癌变组中高于未癌变组(P<0.05)。对未癌变组和癌变组Stat3和C-erbB-2在2年前后的表达进行观察比较,未癌变组2000年Stat3和C-erbB-2的表达均有升高,但与1998年相比,差异无统计学意义(P>0.05);癌变病例Stat3和C-erbB-2的表达均有升高,与癌变前相比,差异具有统计学意义(P<0.05)。Stat3和C-erbB-2在癌变组2年中由阴性变为阳性的比率分别高于其在未癌变组中的比率(P<0.05)。对进展为食管早期癌的17例标本中Stat3和C-erbB-2的表达进行相关分析,存在正相关(P<0.05)。结论Stat3和C-erbB-2基因在食管癌变演进的过程中可能起一定的作用。

Objective To observe the expressions of Stat3 and C-erbB-2 genes dynamicly and to explore their roles in the process of esophageal carcinogenesis. Methods Biopsy specimens were collected from 57 cases with esophageal epithelial dysplasia detected in endoscopic surveys in 1998. The 57 cases were followed up for two years and second biopsy specimens were obtained in 2000. Of them, 40 cases remained the dysplasia of the esophagus and were defined as non-carcinogenesls group. Other 17 cases developed into early stage carcinoma and were defined as cancerous group. Stat3 and C-erbB-2 proteins were detected respectively in non-carcinogenesis group and cancerous group by immunohistochemistry（S-P method）. Results The expressions of Stat3 and C-erbB-2 were not statistically different between noncarcinogenesis group and carcinogenesis group in 1998（P 〉 0. 05 ）. The expressions of Star3 and C-erbB-2 in carcinogenesis group were respectively higher than those in non-carcinogenesis group in 2000, and there was significant difference between the two groups （P 〈 0. 05 ）. The expressions of Stat3 and C-erbB-2 before and after two years in carcinogenesis group and non-carcinogenesis group were respectively tested and compared. The expressions of Stat3 and C-erbB-2 in 2000 were higher than those in 1998 in non-carcinogenesis group, but there was not significant difference（P 〉 0. 05）. The expressions of Star3 and C-erbB-2 in carcinogenesis cases were higher than those in pro-carcinogenesis cases, and there was significant difference（ P 〈 0. 05 ）. The ratio of variation from negative to positive expressions of Stat3 and C-erbB- 2 during two years in carcinogenesis group was respectively higher than that in non-carcinogenesis group, and there was significant difference between the two groups（ P 〈 0. 05 ）. The correlation of the expressions of Stat3 and C-erbB-2 was analyzed in the 17 cases progressed to early esophageal cancer, and there existed positive correlation between both of them（P 〈 0. 05）. Conclusions Star3 and C-erbB-2 gene may play a critical role in the process of esophageal carcinogenesis.
