摘要
了解侵入颅内颅骨肿瘤的临床表现、放射学表现和治疗方法,增加对此类疾病的认识,减少误诊率,提高诊疗水平。材料和方法:搜集1986-1996年间经手术和病理证实的35例颅骨肿瘤(包括肿瘤样变),观察分析其临床表现、X线、CT和MRI放射学表现,并评价手术、放疗对预后的影响。结果:术前X线平片、CT、MRI三者定位准确率分别为93%、95%和100%,综合定位准确率100%,定性准确率91%,误诊3例,占9%。结论:由于肿瘤生长部位不同,临床表现也各异。头颅X线平片、CT、MRI三者结合综合分析对颅骨肿瘤具有定位定性作用。X线平片可作为颅骨肿瘤诊断的首选检查,MRI则能清楚显示肿瘤的生长方式以及肿瘤与邻近解剖结构的关系。手术治疗效果好,部分肿瘤术后放疗可加强疗效。
Learning the vlinical manifestation, the features of radiology and the treatment of the tumor ofskull protruded intracranial, will add knowledge about these diseases and improve the level of diagnosesand treatment. Materials and methods: Report 35 cases of the tumor of skull (include tumor - like lesion of the skull) which were undergone poeration and confirmed by pathological fibdings between 1986and 1986, analyse their clinical manifestation, X-ray, CT and MRI, and discuss the effect of operationand radiotherapy on the prognosis. Results: The accuracy fates for localization of X-ray, CT and MRIare 93%) 95% and 100% respectively. Combination analysis of them results in 100% accuracy for localization and 91% for the exact nature. 3 cases were misdiagnosed(9%). All the patients were undrgoneoperation and their symptoms were reliefed with different degrees or even disappeared. Skullbase tumors(except tumorlike lesion) were undergone radiograpy after operation which is help to improve the prognosis. Conclusion: the clincial menifestation of tumors are not similar because of their different positions. Combindion analysis of X-ray, CT and MRI can detetrine the location the nature of the tumor ofthe skull. X-ray is the first method of choice nd MRI is clear in demonstrating the growth style of thetumor and its relationship to the surrounding anatomic structure. Operation is quite dffective, postoperative radiation therapy used to some tumors can improve the effect of treatment.
