白内障超声乳化吸除人工晶状体植入联合玻璃体切除治疗增生性糖尿病视网膜病变 被引量:8

摘要 目的探讨白内障超声乳化吸除折叠式人工晶状体植入联合玻璃体切除术(以下简称:联合手术)治疗增生性糖尿病视网膜病变(PDR)的有效性和安全性。方法回顾性的分析1999年6月至2006年12月在我院施行联合手术的PDR患者31例(32只眼)。术后随访8~58个月。平均26个月。结果术后视力提高有25只眼(78.13%);视力不变7只眼(21.9%),视力下降1只眼(3.2%)。术后主要并发症包括角膜轻度水肿6只眼(18.8%),前房渗出5只眼(15.6%),玻璃体再出血5只眼(15.6%),视网膜脱离3只眼(9.4%),新生血管性青光眼2只眼(6.3%),术后6个月~2年后囊膜混浊12只眼(37.5%)。结论联合手术治疗增生性糖尿病视网膜病变是安全有效的方法,术后视力恢复快,无明显的并发症发生。 Objective To observe the safely and efficacy of phacoemulsification and foldable intraecular lens implantation combined vitrectomy to treat proliferative diabetic retinopathy (PDR). Method Retrospectively analyzed the clinical date of 31 patient (32 eyes)with PDR who underwent combined surgery during June 1999 and December 2006. The follow-up period was 8 months to 58 months with the average of 26 months. Results The PDR include 31 cases ( 13 males and 18 females)32 eyes with diabetic history for 6 -20 years and eye symptoms for 2 - 10 months. Postoperatively visual improvement was achieved in 25 eyes(78.13% ) ,Visual acuity remained unchanged in 7 eyes(21.9% ) and decreased in one eye (3.2%). Postoperafion complications consisted of a mild corneal edema in six eyes ( 18.8% ), anterior chamber fibrin exudation in 5 eyes( 15.6% ) ,vitreous hemarrhage in 5 eyes( 15.6% ) ,retinal detachment in 3eyes(9.4% ) ,neovascular glancoma in 2eyes (6.3%). Condusion Phacoemulsification and foldable IOL implantation combined with vitrectomy in PDR patients is a safe and effective approach , and provide repid visual rehabilitation without significant complications in the majority of selected eyes after surgery.
