高眼压状态下青光眼手术治疗

Trabeculectomy with paracentesis under the condition of high intraocular pressure

目的观察高眼压状态下施行青光眼滤过性手术的疗效。方法对33例持续高眼压性青光眼患者先行前房穿刺缓慢放出房水后,再行小梁切除术,术后观察前房,滤过泡及眼压、视力变化情况。结果33例高眼压下施行青光眼手术的病人恢复顺利。术后一月眼压控制率93.9%,观察2年以上,眼压控制率84.8%。术后视力均有不同程度提高。无严重并发症发生。结论高眼压状态下前房穿刺后再行小梁切除术是持续高眼压性青光眼治疗的有效术式之一。

Objective To observce the effects of trabeculectomy with paracentesis under the condition of high intraocular pressure （IOP）.Methods Paracentesis was taken to reduce IOP gradually, and then trabeculectomy, tamy was taken for 33 eases, Filter bleb, IOP, visnal acuity and anterior chamber condition were observed postoperatively. Results 33 patients with persistent high IOP convalesced after operation. Postoperative cases with IOP〈21mmHg were count 31eyes （93.9%） by a month. The IOP control rater was 84.8% after two years. Visual acuity was improved in 33 postoperative eases. No serious complications were found. Conclusion Trabeculectomy with paraeentesis can safely reduce IOP and keeps an acceptable visual acnity, so it is an effective method for glaucoma with persistent high IOP.
