摘要
目的:探讨全血B-型利钠肽水平、左室射血分数、心电图V1导联P波终末电势、心胸比在评价心力衰竭患者心功能的临床价值。方法:采用心力衰竭诊断仪测定心衰患者全血BNP水平,同时采用NYHA心功能分级和超声心动图评价患者左心功能,常规描记十二导联心电图并计算PtfV1值,在标准胸部正位片上测量并计算心胸比,对结果进行相关性分析。结果:在不同临床心功能分级之间,全血BNP水平存在显著性差异(P<0.05),BNP水平与左室射血分数呈负相关(r=-0.628),与心电图PtfV1值呈负相关(r=-0.756),与胸片心胸比呈正相关(r=0.757)。结论:测定全血BNP水平,结合NYHA心功能分级、超声心动图左室射血分数、心电图V1导联P波终末电势和胸片心胸比有助于对心力衰竭患者的心功能进行综合评估。
Objective: To investigate the relationship among blood B-Type natriuretic peptide (BNP) levels, left ventricular ejection fraction (LVEF) , P-wave terminal force-V1 ( PtfV1 ) , cardio-thoracic ratio (CTR) in patients with heart failure. Methods : Blood BNP levels were determined by BNP triage test device, LVEF were examined with echocardiogram, PtfV, were measured on electrocardiogram and CTR were measured on X-ray film in 32 patients with heart failure as well as 23 controls, then made a statistical analysis for the results. Results: The blood BNP levels elevated with the worsening of heart failure ( NYHA classification), with significant differences among the patients with different cardiac function grades ( P 〈 0.05 ). The blood levels of BNP were negatively correlated with LVEF ( r = - 0. 628 ) and PtfV, ( r = - 0. 756 ) , but positively correlated with CTR ( r = 0. 757 ) . Conclusion : Measuring the blood BNP level, combined with NYHA classification, LVEF, PtfV, and CTR, we can obtain a comprehensive assessment of left cardiac function in patients with heart failure.
