摘要
目的:总结分析移植肾功能稳定期与急性排异反应期二维及彩色多普勒血流显像特异性表现,以期得到超声诊断移植肾急性排异反应的有价值指标。方法:对119例移植肾功能稳定期与46例移植肾急性排异反应期患者的长径、宽径、厚径及实质厚度以及主肾动脉及弓状动脉的收缩期最大峰值流速、舒张末期流速、平均流速、阻力指数、搏动指数结果进行统计学分析。结果:两组比较移植肾长径、宽径、厚径及实质厚度有统计学差异(P<0.01),急性排异反应组肾内血流灌注减少,达不到皮质边缘处,肾动脉血流速度收缩期不明显,而舒张期明显下降,搏动指数、阻力指数明显增高,与移植肾功能稳定期比较有统计学差异(P<0.01)。结论:肾锥体回声减低、舒张期肾动脉血流速度下降、阻力指数和搏动指数升高对诊断移植肾急性排异反应有特异性。
Objective: Tailing up its particularity performance through analyzing different reaction of two-dimensional and Color Doppler flow imaging(CDFI) performance during renal transplantation function sta- bility period and acute rejection period, in order to get valuable index on. acute rejection of renal transplatation. Metheds: The results of 101 renal transplantation function stability period and 38 acute rejection period patients after B-mode and Color Doppler flow imaging examination carry on .statist ics analysis. Results: The 2-D images(length.breadth and thickness) of acute rejection period patients were larger to those of the normal group, and cortex thickness became thicker than nomal group (P〈0. 001). In the acute rejection group: Marginal blood vessels in renal cortex were lesser. The blood velocity was decreased only at the diastolic phase. Pulsation index (PI) and resistance index (RI) were higher than those of the normal (P 〈0. 001). Conclusion: CDFI is an important diagnostic tool following acute rejection of renal transplantation.
