The Clinical Application of Magnetocardiography to the Coronary Heart Disease Patients in the Following up Coronary Stenting Results

摘要 目的:探讨心磁图法对冠状动脉支架置入术后随访的临床应用价值。方法:应用安装于非屏蔽室内的7通道心磁图仪,检测61例冠心病患者支架置入术前、术后24h及术后1个月的心磁图,分析图像中有无具相应幅度的电流矢量的额外区域,并将这些区域存在的强度和时间与支架置入术前、术后不同时间进行比较,分析两个定量指标:①异常磁图比(ratio of abnormal maps,RAM):异常的心磁图所占的百分比;②平均分级(average classification of total maps,ACTM):ST-T间期所有磁图数的平均分级。结果:支架置入术后24h的RAM(65.92±29.01)%、ACTM(2.96±0.58)与术前RAM(72.46±38.77)%、ACTM(3.03±0.51)相比,均无显著差异(P>0.05);支架置入术后1个月时RAM(50.62±29.77)%、ACTM(2.41±0.34)显著低于术前参数(P<0.01)。结论:心磁图是冠状动脉支架置入术后随访及疗效观察的一项很有前途的无创性检查方法。 Objective:To study the value of the Magnetocardiography (MCG) clinical application to the coronary heart disease (CHD) patients in the following up coronary stenting results. Methods: Sixty-one patients with CHD selected for coronary stenting were studied. MCG measurements were performed before, 24 hours and one month after coronary stenting by means of the seven-channel magnetocardiographic system (MCG7,SQUID AG,Germany) ,installed in an unshielded room. We analyzed whether there showed additional areas of current flow vectors in the perfusjon area of the affected coronary artery in the MCG mapping. We compared the lasting time and intensity of the additional areas before, 24 hours and one month after coronary stenting. Two quantitative indicators were analyzed:①RAM: the ratio of numbers of severe abnormal maps over total maps;② ACTM: Average Classification of Total Maps. Results: No significant difference was found in RAM and ACTM between before[RAM: (72.46±38.77)%,ACTM:3.03±0.51] and 24 hours after coronary stenting [RAM: (65.92±29.01)%, ACTM:(2.96±0.58)](P〉0.05). RAM(50.62±29.77)% and ACTM(2.41±0.34) one month after stenting were much lower than that of before stenting [RAM: (72.46±38.77)%, ACTM: (3.03± 0.51 ) ] (P〈0.01 ). Conclusion: MCG mapping is a promising non-invasive method of evaluating the effects monitoring to follow up coronary stenting results.
