摘要
目的探讨青年人大肠癌在临床表现上恶性程度高的原因。方法采用免疫组织化学和流式细胞术同时检测经手术切除的癌组织P53蛋白的表达。结果免疫组织化学染色结果发现青年组P53蛋白表达阳性率明显高于老年组(P<0.05),流式细胞术定量分析亦显示青年组P53蛋白表达量(FI=1.34±0.26)显著大于老年对照组(FI=1.15±0.25)(P<0.01)。结合病理学进一步分析发现:在青年组中黏液腺癌和浸润周围软组织者的P53阳性率(100%,95%)均分别显著高于腺癌和浸润未达周围软组织者(67%,58%)(P<0.05)。另外,与老年组比较,青年组在黏液腺癌和癌浸润至周围软组织的发生率等2方面均明显升高(P<0.05),同时相应的P53蛋白表达阳性率亦是如此(P<0.05)。结论P53蛋白过表达与青年人大肠癌组织学类型和深度浸润密切相关,可能是造成青年人大肠癌恶性程度高的原因之一。
[Objective] To investigate the cause related to high malignance of young patients with colorectal cancer (CRC). [Methods] Immunohistochemistry and flow cytometry were utilized to detect the expression of P53 protein in CRC tissues obtained by operation. [Results] The positive rate of P53 expression in young group was significantly higher than that in elder group by IHC (P 〈0.05). Through FCM quantitative analysis the positive expression of P53 protein of young group(FI=1.34±0.26)was also significantly higher than that of elder group(FI=1.15±0.25) (P 〈0.01). Further pathological analysis indicated that positive rates of P53 expression in mutinous adenocarcinoma (MAc) and in cancer with invasion to surrounding soft-tissue(SS) (100%, 95%) were markedly higher than those in adenocarcinoma (Ac) and in cancer without invasion to SS (67%, 58%) (P 〈0.05) in young group, respectively. In addition, pereentagse of MAc and invasion to SS in young patients were significantly increased as compared with those in elder patients, respectively (P 〈0.05). Meanwhile, also similar results were the corresponding P53 positive expression. [Conclusion] P53 overexpression was related to MAc and deep invasion of CRC in young patients, which might contribute to high malignance of CRC in young patients.
